If they would have put that warning on chapter 1 it would have spoiled it as many people were shocked/surprised on what happened there. They also didn't go into details on the rape scene in the first chapter unlike chapter 11 in which they show a lot of details of what's happening to Kyungsoo
yea if they said there was a disturbing scene in chapter one then yes, it would have been a spoiler, but they could have specified before first chapter that in the manga in general there would be something. like, so we don't know to who or when it would happen. just that it is there.
