Have you tried Harada's works?.. Yajirushi, Yoru to Asa no Uta and Nii-Chan pretty much shows the impact of the said trauma but the ending for Yajirushi and Nii-Chan isn't really ideal.. it's probably not what you're asking for but it does show the impact and the affects of it..
You can also try Lost in the Cloud, one of the main characters went through SA as a child which affected his character quite significantly and did show his healing process!
I think Pearl Boy also counts... If you haven't read it already.. cus it's really popular... haha.. I think it also showed one of mcs healing process and the perpetrators did get karma.
Obey Me.. again if you haven't already, it's popular too... While Obey Me did show how it affected the MC, he couldn't get away from the perpetrator in the end.. I'll try to look/remember 4 more... But for now this is what I have! (⌒▽⌒)

yeah ur right, the bl didn’t romanticise anything & joohas perpetrators were clearly portrayed as villains. but in the middle of the story, dooshik has these perpetrators raped as a form of revenge. yes, jooha himself was a victim of SA but using SA as revenge is problematic. a lot of ppl think that dooshik was in the right for doing this but he isn’t. it depends on what op is looking for tho.. mayb they’ll like it lol

BLs are pretty well known for having a lot of SA, but I’ve only found one (Ouroboros) where it doesn’t brush it aside.
