Its not from stress, it's from his brain not being able to process having memories from his real life outside the novel as well as the memories of the character he was transmigrated into. Basically just his brain is overloading, that's why he gets the blood noses, vomits blood, passes out for what appears to be no medical reason. It explains it better in the novel

Its actually from injuries he suffered back in his original world where he took days to die under a collapsed building
Six chapters in and I’m holding back tears as I watch him seize from pure stress
Not sure if I can keep reading, nothing wrong with the story at all but this hits way too close to home and the author/artist portrays it well.
