I don’t know, his stitches almost look like tattoos because of how detailed everything else is and how flat they are. Like, is it really just an oversight the artist draws every dip and shadow of his muscles and NO movement of the ligature being tugged when in motion. Also, stitches are REMOVED after a period of time, they don’t stay in. So he would in fact have to be a corpse if these are holding his vital areas closed/together (neck, wrists, potentially ankles). But this ALSO would indicated everyone around him knows he’s undead and that there’s a different kind of maggot. Which has not been acknowledged (not a single person including the MC has mentioned the stitches or “injuries”) therefore does not seem to be the case? So I think they are just tattoos.
My theory is that Guwon is dead already but thank to some frankenstein experiment he’s alive that explains his sewed body and childish personality.
