He's the one that created them all...he also created Ishino as a way to control them but when they all escaped he wiped Ishino/Clavis's memories of the lab... the doctor is a sick f*ck he even experimented on himself replacing some of his own body parts and enhancing his voice so Ishino and Karga had follow his orders with the help of an implant... it's all kinds of messed up...
