It's only unclean when it comes out after the urethra because that's when it's introduced with bacteria, germs etc on the outside. Even without peeing inside just having sex is enough to give UTI to a woman. Bacterias can easily crawl into the urethra from the genital and anal area. This comes from a student nurse by the way and as a person who actually got a uti bc of it

No, it's not. It is sterile when it first leaves the body but that doesn't make it sterile forever. It was used to clean clothes because of the ammonia in it, doesn't mean it didn't also stink to high heaven because of the bacteria that also will grow in it minutes after it has left the body. Douches also cause UTIs in women, urine is worse because it is more of a vector for bacteria and it kills more of the native microbiome in the vagina that prevents UTIs.

I absolutely don't have anything against piss kinks but absolutely do not ever piss inside of a vagina that is unsafe.

With your logic, it's still exiting his urethra that has the germs on it, how would that make it safe? Urine still grows bacteria in it minutes after it has left the body, meaning the body it originally came from, still being in a uterus will not keep it sterile, not to mention the ammonia in it will destroy the microbiome much like a douche does which has been scientifically proven to increase the rate of UTIs. No one should ever do this.
