I'm not really sure, this is just what i think but the panel where it's showing his last flashback, i think he did let go and not pull his father away ,like he just let his father be hit by the passing car
Nah that was just his dreams. The father died running into the street and getting hit by a truck
In Chapter 69, he does run after his father when he runs into the street. So there is a chance that he got to hold his father's arm for a moment and then let go. So he would feel the most guilty for that

Then what happened to him? Why he is so injured?
Well his dad did break a bottle in his head and he’ll fell on the floor headfirst too and if I remember correctly there was a car behind him at the same moment the truck was heading straight into his daddy too so I’m pretty sure he got hit too
