Maybe depression and anxiety
considering how much he drank, wouldn’t be surprised. but if his liver was failing, he wouldn’t be walking into the ocean, he’d be in pain my guess is depression from childhood abandonment, and he’s afraid his grandma is gonna die and leave him alone.
Praying for something curable
Ok, so my totally educated and professional guess is organ damage/failure of some kind. Although liver's usually is accompanied by jaundice and he doesn't look yellow so maybe kidneys?
