Okay let me explain. First of all, if you go back to when Major Kwak was having a dream about his childhood, that was when he wasn’t a citizen and was living outside of the city’s care. Those people aren’t protected by the city because they aren’t citizens and Rainbow city values worth of people more than lives. In dr.Seok’s flashback when he was a kid you can see others in the training center with him, I can only assume those are also citizens.
The vaccine is a huge deal because the virus keeps mutating making vaccines useless so new vaccines have to be made and it’s a huge deal because rainbow city officials run the government and the garden of Eden are rebels. So imagine the rebels making a vaccine before the government. (Not a good look)

Also the 4 ppl talked about are the elders or the 4 founders of the garden of Eden. In the Bible the garden of Eden had 4 rivers hence the Gihon, Tigris, Euphrates blah blah.
Rainbow City is a city not a country. And I think that people behind the Garden of Eden are people from rainbow city who rebels from it. I also think that that vaccine was proposed before but the rainbow city did not approved,or maybe they left rainbow city without introducing that vaccine to the masters and elders.
Other Cities are also protected by the military/government,see Jeju island where Dr. Seok came from. Unfortunately for others,their security is not as tight as these two cities,see the place where the young Major Kwak came from. It's still the same corrupt government that rules over poor people.
