Id like to think rather than muscular, he lost all the fat around his body due to the fever and high metabolism. Hes more lean because in the story it's said the fire sprit burns calories and cuz him to have a ton of appetite.
How in the hell did he get that muscular if he has a frequent severe fever? When is he exercising to maintain that shape?
