Those serving royalty also come from noble background that are expected to be high in ranking, the fact that Joffery doesn't care about Dot's family name showed how he doesn't care or mind about status, or even if a commoner become his servant/knight/friend. Which is kinda unthinkable because I'm sure there are many status discrimination in that world, which is why the knight commander was shocked.
