"You're always thinking about complicated stuff, Shuu-chan." Yes, that's the problem lol. For some reason, Shuuhei's mind is blocking the idea of loving Tomoya. In his dreams, he is unable to eat Tomoya. Eating = Loving. I think that means he is afraid to love Tomoya, maybe because he's afraid of the possibility of losing him when he dies. The ex-girlfriend's story about her cat that died is totally their story, with Tomoya as the cat.
In chapter 2, Shuuhei asked Tomoya why zombies eat people when they don't need to do it to survive because they're already dead. Tomoya said that maybe they want to regain the soul they've lost. Shuuhei is like a zombie. I think he lost his soul when they lost contact after middle school. But he kept looking for a way to eat Tomoya in college. He dreams about eating Tomoya. He saw Tomoya in his ex-girlfriend and both ex-girlfriends know that he loves someone else, "There's someone inside you..."
Shuuhei doesn't really need to love to survive, but by allowing himself to love Tomoya, he will feel alive again.
That's just my theory/interpretation. I could be totally wrong, though lol. I love how this manga makes us all think.
