I'm guessing it's using hard sex as a punishment to yourself. Sex isn't all pleasure so I can see that happening
The mc participates in the act of boombayah because he's doing something that's he knows is will be painful, over and over again, because he feels he deserves the pain. Its a form of self harm because he's doing it to punish himself, and does it in ways that he knows will hurt him most.
Anything can be self harm tbh because it's anything done to the detriment of yourself. It entirely depends on the reasons you're doing it.
