Try as I might I've not been able to find this one.
It's a Korean BL, I'm pretty sure it's younger top x older bottom? College/modern setting. The twist was that the younger top looks "older" (it was a singular wrinkle under his eye which turned out to be a scar LMAO) and some of the bottom's classmates see them and spread rumors that the bottom has a sugar daddy. Also I'm fairly certain both the bottom and top were lowkey kinda insane or need therapy, plus the top was a batshit yandere? Or just generally batshit?
Other miscellaneous details I remember is that the top might've been a rich kid, and also the top got revenge on the guy that spread rumors about the bottom being a sugar baby (which ruined the bottom's life for a bit) by basically selling the dude to some gang/mafia??? It was weird and I need to figure out wtf was happening lol
