Its an illusion, not real. This has 7 chapters. In the end, he (MC) saves him (dragon) with a potion that the sage fairy makes for him. Then after the dragon recuperates they get married, dragon uses his double penis to impregnate him during their honeymoon sex and the tattoo mark glows and turns into a dragon egg (guess he doesn't get preggo like some mpreg) and they have a beautiful baby boy that looks like our MC.
