I knew half way somehow kou was connected to shirou's grandpa but I didn't think that they'd be in that sort of relationship, then and there it could be easily interpreted as kou still being in love with kikujirou with unresolved feelings which was not addressed properly but also expected it that it's gonna open ended but not to this extent!
