this is just what i understand, i think they traveled back in time. the uke thought he got isekai-ed into the novel, and the seme in the beginning is the novel-seme (the seme in the novel). then uke got into therapy, yada yada and when he woke up he feels like the seme has changed into his original seme (means he feels like seme from his world got isekai-ed into the novel as well) uke discarded the thought and just think of getting the happy ending for the novel so the novel-uke and novel-seme end up happy. now the seme is giving us hints that there's no such things as isekai, it seems like both of them come from the future so they can fix their past. seems like seme just got his memories halfway that's why he changed from an asshole to nice guy
it's not confirmed btw it's just my own conclusion
