Because the empress saw the baby luana and her brother at the concubine palace when they were both baby. The empress’ baby died a year prior. So it’s impossible for her to notice it. The Emperor is a distant relative of the royal family so it is possible for a baby to look like luana even if she didn’t get the traits from both parents. The concubine used a spell to slow down the growth of the child. That’s why it was still a baby after a year.
Why does she keep denying that she could be the empress's child??? It doesn't make any sense and not only that the people around her also think it, like - its clearly obvious that her mum is the empress they look like exact same. Lilliana doesn't even share the same hair colour or eye colour. All three of them have the same power although at different levels is evidence enough that luana is the empress's child, like WTF IS THIS?????
