If he sees a cut on them or something he kills them, and i dont think he actually checks if they are infected/not infected yet, he just kills them cs i think its very high chance of getting infected if u have an open wound of any kind(this might be wrong sorry)

It's never explained. We just know he has some incredible innate ability to sniff out xenogenics and has a 100% accuracy rate. I think his experience is supposed to parallel An Zhe's. An Zhe is an anomaly who learns what it means to be human. And Lu Feng is a human who is treated like an anomaly.
