It feels like a wat if scenario of wat would happen with them in different circumstances, I think mc in that universe never got into an accident n had a successful career n never ended up dating red head. N the dream ends with mc n ml fucking n maybe getting together(?). In the end its revealed its a dream, mc wakes up n ml is there with him. But yes its like an alt universe.
