It's the father, he have to take meds, and he have problems with his behaviour and his mind. Also, when Shizuma is back from his absence, he shows to nurses photos, and there are albo photos of Minato in a dress. The all tlaks, that he looks gorgeous, but doctor ask Shizuma if he doesn't feel disgust when his boyfriend wears woman clothes, and what he would do, if Minato would said one day, that he wants to change his gender. Shizuma is saying, that he is not the ine who could choose, and that the most important is to feel good with own body, bacause otherwise it would be too paintful. Those words gave shock to Doc. (Sorry for my bad language, I'm not use english very often, but I wanted to help a little, because 5 volume is already out in my country).
I had read the raw.. Turns out the director change his appearance as his own sister because his father/grandfather is kinda sick.. It's like his sister is the beloved child in their family tree. And obviously most parents are unable to comprehend well when they beloved child died etc. Hence he decided to being the 'dead' one instead of his sister
