So far from what i've read in the novel, both the emperor and empress love each other dearly. But the emperor is keeping the empresses at arm's length coz he knows he's sick and might die soon/dying. But when the emperor awakened (with the help of yuder, kishiar and the empress) he became healthy and was able to be more affectionate with the empress.
Also they became aware if Kishiar's relationship with Yuder and they are accepting and treasures Yuder as well. :) The emperor even expressed dismay that Yuder's bright future might become difficult because Kishiar has absolutely no intention of hiding it
