I know sometimes in other places highschool students can be 18 and over (its not where im from so im never sure) and boy, he don't look like an adult whether thats due to his scrawniness and bad shape or because he literally isn't one!

Not true at all malnourishment stunts so much in a persons growth. He was in a abusive household so of course he looks scrawny, its not like they had that much of a height difference, he was just slouching.
ofc you could say he’s malnourished as well but given the complete size and structure difference of his body than and now, it really cannot only be that. unless there was a hardcore training montage we haven’t seen yet, he’s so very clearly a child there. at least, visually. but who knows what the author’s gonna say.
