It has been told to him what to do it's now left for him to make the effort to leave that particular spot he's in
Yeah, it starts from within. But the kind of mental problems that he has can't be solved without professional help. it's unrealistic to think otherwise. He needs the necessary tools to confront his problems.
And I speak as a person that suffered depression and still has anxiety.

The doctor should have referred her to a psychologist, not told him to try to figure out what was wrong on his own.
