I think why he is still taking it even when he knows it's wrong and dangerous, is that he's never went against her words before (mentally distorted possibly? ) so he want to leave her but when she there he literally does the opposite of his plan, which shows how much of an upper hand she has (also she had to literally force feed him the medicine mama bird style when he didn't take it（￣へ￣）)
He still needs to make appearances with the queen. In order to fully keep her under pretense of his plan, he has to keep listening to her. It didnt help too that she was alr suspicious of him and shar when shar visited the annex when she was away, so the pressure is there. But the other factor is that its not that easy to shrug off years of brainwashing easily. Esp with how tight the empress manipulation with his mind.
