He realized he was betrayed when he was drugged. But in the recent chapters, it hasn't shown that Heesung realizes the part where he was manipulated into believing that his hyung treated him as family.

His hyung talked shit about him in front of him as well and even wished it was a dead animal instead of finding him alive back when he was abandoned by his parents ;-;

In the next chapter, he managed to get away and was later found by Yoon Chi-young.
The next chapter after that, Yoon Chi-young brought Gyeon Heesung home were he nursed him back (since he was drugged). When Heesung realized where he was, he hid inside the closet. Chi-young came home and knew he was hiding inside the closet. Heesung, attempting to run away as soon as Chi-young opens the closet, bolted out and smacked him yelling he have to get out of his house. Chi-young caught his arms, resisted, then went and kissed Heesung.
It was adorable and satisfying to see Heesung is back to his human form and we see Chi-young dotting on him like he always did to his puppy form ( ꈍᴗꈍ)
