I haven’t read it yet but having twins as a teenager will not kill her just because they’re twins. I know a girl who had twins as a teenager and she’s absolutely fine and another with triplets.

That author has never gotten pregnant or is simply very much ignorant and it shows cuz even a teenager knows having twins isn’t lucky it's twice the hardship in labor. If it were the real world Henrietta would die after her pregnancy 100% and Rainer would be one of those fathers who blame their children for the mom's death
