I only like it when they put in it the comedy kind of way. Other than that, it's a bit overboard. I'm currently reading manhwa 'Raise me' where the 'older' giy is only 25 (he's younger than I am lol) and his partner is 20. The way 25 is portraited, I just can't. He refers to other guy as 'fetus' because he's so so much younger and he's wondering how things were in his days aka when he was young. But it's romantic comedy, so it can be tolerated pretty much. It adds to funny element a lot.
I like to think of it more like the older (30+) men being portrayed are usually office workers (at least in most of the manga I’ve read with an older man), and along with being an office worker, they usually are the stereotypical work only-no exercise or breaks type of character. If just sitting at a desk for years and years, then you’ll definitely start wearing down physically by the time you’re in your 30s. Otherwise idk maybe some authors like playing up the trope of old man=back pain, etc. lol
As a 39-year-old and someone who’s around people in their late 30s and early 40s, I can say YES, this age feels old, though not "VERY" old iykwim. This is the age where all those late-night parties and long hours sitting without exercise start to take a toll on you. It’s also the age where your sexual drive is either overactive or decreasing—nothing in the middle. For men, this is the age where pancreatic issues can become more common, and for women, it’s often the pre/menopausal stage. So yeah, there are definitely effects on the reproductive system. It doesn't start at 50 or 60. It can start as early as late 30s. Like you said, unless they are healthy and have no mental/psychological issues, but most of the stories we read (like this one) they are either stressed or a busy salary man so obviously they dont have that healthy lifestyle hehehe.
Also, nowadays we see 40-year-olds looking young and having a youthful mindset, but there are also 39-40-year-olds who already have grandchildren. Yea so it's true we can be grannies or gramps already lol

But I have genuine question, what is with manga/manhwa authors portraying men between 30 to like 45 as old grandpas?
Reproductive organs at those ages work absolutely normal. If man is healthy and take care of himself, plus doesn't have any psychological symptoms that can cause erectile dysfunction, his pp can 'work' normally until real old age like over 60 etc.
It's kinda irritating whenever such ed trope appears because one partner is younger than 30 and other is around 40, he just magically needs digestion medicine, glasses and viagra. Also he usually can't even exercise anymore because his 'old bones' hurt.
