That's what we call intrusive thoughts. Intrusive thoughts that aren't quirky or simple impulse actions. What he thought to do to Myeongi are intrusive thoughts. It's disturbing or scary and yes, it need help If it gets too visual and it's taking control but normally intrusive thoughts are like this. Doing something horrible to a person or to yourself.
Which is also A bit sad for Heonjea. Since he clearly needs mental help but was brushed off or called out weird and was bullied for it.
But not all violent thoughts are intrusive thoughts
