To make things easier I’m just going to refer to them as alpha(A) and omega(O) I think what happened was is that the O had been taking care of the A since he had awakened and the A has had feelings for the O since they first met, and with the continued exposure to the A’s pheromones it triggered the latent Omega genes in the O’s body. But the O never thought anything of it cause the symptoms of the O’s heat matched those of a common cold, and during one of the A’s ruts “the part where the story started and the O was giving the A his suppressants” and this triggered the O’s instincts and well they had sex and then the O got pregnant
