I think this is a perfect example of an unreliable narrator.

And I say that in the best way possible. The plot twist was so nice because up to chapter 4 we weren't even given the opportunity to doubt the uke. And it is not like he was lying or making up stories to become an unreliable narrator, he simply was not in the right state of mind. His brain couldn't handle all that trauma and resorted to forgetting everything, thus making such an interesting story with having the uke think he has been kidnapped by a phycopath.
And with what we are shown the seme is truly a green flag!!!
Although I would have loved if it was longer,aybe I don't know a continuation of those two interacting while they are both with a shame mind, but still a very great read, I would recommend this to anyone.
Also I love the detail that up till he remembered everything, so up to chapter 4, the ukes appearance is of him as a 20 year old, because that's what he remembers, while his scar is shown only when he truly remembers everything and in chapter 5 where the narrator is the seme.
