I'm guessing based on Freya's reaction to Cesare kissing Ruby ; the rumor should be about an incest relationship between him and Ruby. But maybe a novel reader can expand on that thought
My guess would be, he just spread some defamation rumors in general about her, for example lies about her marriage partners and stuff. For example her Ex was defamed by cesare so he couldnt marry ruby in the end because of impotence
