Yeah, Hayden explained it in one of the very first chapters I think. When other alphas sense another alpha's pheromones its more like a pressure they feel in their brain. So when Hayden is in his rut and releases his pheromones, I imagine it's more intense than usual and it would cause Brett immense pain, that's why his nose starts bleeding during sex on some scenes.
