I don't think such thing will ever happen in the manga but I have two theories.
First theory: he won't be able to transform into a man while being pregnant in the female form
Second: He can trasform into a man, and with the yaoi logic, he's gonna grow some sort of male uterus (yk how they make pregnant males in omegaverses? Yeah that thing), but let me specify, it's gonna be the same uterus that he had in the female body because that's where the baby is.
Of course the second theory is less likely to happen, but this is a fantasy manga (the idea of someone switching genders is unrealistic), so it is possible

Yeah, the second one looks really yaoi-like, I was thinking about this one. I also have a theory, that baby would be born prematurely because there wouldn't be any space in male body. It would be a long transformation as baby would have to escape the body or sth similar to C-section maybe?
Most likely of course his reproduction system is not working properly because of drug, so he wouldn't get pregnant. The case popped my mind because they didn't use protection

If Shion or Hodaka would have baby in their female form, but then turn into a man, what would happen to baby?
Serious question, I don't take "He probably can't have babies in female form" as an answer.
