There's 5 chapters (plus an extra). Chapter 4 is not the end. Here's the Spanish version https://visortmo.com/library/manga/75376/itsukanotrueend

Sure [SPOILERS] *warning for anyone that wants to wait instead of being spoiled*
You can also use the google translate app if you have it in your phone. It has a camera feature that translates whatever your camera is showing at real time or whatever pictures your take.

    Chapter 5:
Page 1-2: Starts with Rei seeing Yamato off as usual. Rei is told by one of his subordinates they still haven't found a way to break the curse that was cast on Yamato.
Page 3-10: Months later Yamato told Rei he was getting a leave of absence from school. Yamato wants to spend his remaining time left with Rei. Rei starts reminiscing and telling Yamato about how much he hates humans due to the war and how he hated Yamato too at first. He went along with marrying Yamato to get peace for his people. Rei didn't care since human life is fleeting so the marriage wouldn't last long (to him). But now he is afraid of losing Yamato and doesn't know what to do. Rei blames himself for Yamato being cursed and he's hurt at the thought of losing Yamato.
*Note the order at some of the panels particularly pages 5-6 of the manga is a little weird. I don't know if it was an error from the translators or not. Basically while Rei is telling Yamato how he initially didn't like him Yamato says things that don't make coherent sense such as "Wanna go for a drink?" "I like these types of places"
Page 11-17: Now Yamato is the one narrating. He tells Rei he loves him after he stops himself from telling him he's sorry (for making Rei sad). He wants Rei to be sad that he's dying. And they have sex
Page 18-27: Demon subordinate returns to tell Rei that they found a book that includes a legend of a human woman who was cursed (similar to Yamato) and was in love with a demon. According to the legend, when a human and a demon love each other, their souls are connected and the human enters a cycle of reincarnation. The demons explain that from the demon's pov that would be more like a curse since the demon will be destined to always love the human, watch them die every time, and suffer while waiting for them to be reincarnated. (similar to CLAMP's Wish if anyone has read that). Yamato insists he will find Rei once he reincarnates but Rei tells him that he's not going to remember anything and that he won't be able to find Rei because Yamato is going to always be reincarnated in his original world. Rei is doubtful but Yamato tries to reassure him (and himself) that everything will be ok.
Page 28-end: It's after a time skip. Rei is back in the demon castle by Yamato's fresh grave. A lot of time passes. Yamato's helmet that is on his grave rusts with the passing of time. So much time passes Rei stops crying when thinking of Yamato and the memories of Yamato become a distant past. At that moment he feels Yamato calling him (meaning he has reincarnated). Back in Japan a high school reincarnated Yamato gets in a subway train and Rei sits right across from him. Rei narrates the end and says that no matter how painful it is to lose him he is sure he will aways end up falling in love with Yamato. And him meeting Yamato is the true ending for them (the title of the manga).

    Ch 5.5 (Extra):
I think this extra actually takes place Yamato's first life with Rei (not in the life of the reincarnated Yamato). Yamato's university friends notice he's been wearing a ring every day and ask which of the girls ended up being his true love. They ask Yamato to help set them up but Yamato replies that he can't because he has already gotten rid of all girls' contact information. Rei comes into the university to bring Yamato the bento lunch with his homemade cooking Yamato forgot at home. Yamato says he already had lunch (at the cafeteria). Rei introduces himself to Yamato's friends as Yamato's husband. Rei offers one of Yamato's friends the lunch so it wont go to waste. But Yamato gets jealous and says he's going to eat it. Yamato angrily takes Rei to a hotel and tells Rei to never give his homemade food to anyone else that isn't him. They have sex and towards the end Yamato tells Rei that Rei and everything that comes from him belongs to Yamato. Yamato apologizes for being selfish afterwards and Rei tells Yamato that he needs Yamato to be clear with how he feels and what makes him happy since Rei has a hard time understand social cues. At the very end Yamato realizes he's late for his classes.
