So basically the Emperor has always loved the Empress and only her but as usual, the two have a miscommunication issue.
The Emperor initially took in a concubine because he was taught when he young by his father (I say brainwashed in a way) to have a concubine to keep the power in check.
Then the concubine used sort of a love potion on the Emperor so the Emperor becomes horny+obsessed with the concubine. This, along with the miscommunication between the Emperor and Empress, as well as the concubine utilizing the miscommunication to drive a wedge between the two, led to the current situation.
In the original plot line, because FL wasn't there, the Emperor and the Empress kept misunderstanding each other till the end. This combined with the love potion, led to the ML's situation.
In the current timeline, the effects of the love potion eventually fades and yes the Emperor and Empress will end up together. At the ending, the Emperor realizes his stupidness and gives the throne to ML and goes to live with Empress at the Empress's house. Oh and idk if the Empress directly forgives him. I think the Emperor goes to live with the Empress asking for her forgiveness while the Empress say no in a tsundere manner.
This is an assumption only, but I think the effect of the love potion fades because of FL interfering and making the Emperor realize his misunderstandings. Which deepens his love towards the Empress and overcomes the love potion.
Ps. At the end, the Concubine kills her father and brother and then takes poison to kill herself because she didn't admit her sins (of killing father, brother and kidnapping FL)
