i agree 100% considering how much they have been foreshadowing it too and tenju telling kiyotaka he can’t promise not to get injured
I'm almost 100% sure that Tenju will be injured and on the verge of dying. And he will be transported to the local hospital for emergency surgery. This way our cutie doctor will finally overcome his trauma by saving his loved one with his own hands, while the sexy yakuza will be "reborn" and able to find himself and his path outside this little sleepy city.
Well, that's what I'd have written in my story as the most predictable finale
