I dont remember all of the details but is works basically as WeebofLevi wrote. She can reincarnate endlessly. So the novel was her 1st life, then in modern world and next current life.
Wait after reading chapter 70, was Athy always Athy? Like Lucas thoughts kinda hint that she first died by hanging which is how Claud killed athy in the novel of lovely princess, than he said despair which is real world Athy dying after getting sick and taking meds. So like what if all those versions, the novel, real world, and novel again have always been her? Like just in different worlds, like she hopes from one world to another when she dies??
