well, in my understanding, shougo met up with the sister (at the hospital), hence the strong scent (he said that the sister liked to wear strong perfume, if he was talking about the babysitter, that wouldn’t make sense cus how would he know their preferences). as for the child, either shougo or akira probably asked someone to look after him for a short time while they were both at work, or maybe (shougo) left him to the babysitter at the sister’s house and went to pick him up when he dropped the sister off.

But its still unnecessary for the sitter to be at the hospital when she's the one who needs to take care of the kids or leaving the kids to the sister to go to hospital (doesnt make sense), and perfume part as well because of how sensitive kids noses are. Plus in the next volume (spoilerrr)

If you read the unofficial translation, it seems that the sister is the one who went to the hospital but if u read the official one... Its much more confusing and ambiguous but base on my understanding on the official translation....
Sister went to the hospital to talk about the babysitter... Go to the sister's house (drop her off) to see babysitter since the babysitter is already there.
