It's a Mafia term. It means someone who is left to "clean up the mess". For example, if someone higher up accidentally leaves a witness at a crime scene they get whacked. Or perhaps they leave a piece of evidence, they might set the place on fire to destroy the evidence. They're bottom rung, like gofers.
