I think you're confusing with her unfortunate family problems with Al's own wishes... Shes born a women, and she. herself IDENTIFY as a women. She had no choice but to PRETEND to be a men cause of her toxic fam, and the potion her step granny forced on her is POISON that keeps her body like that and the side effects were about to make her BARREN, to reiterate the POISON is not some magical transgender-making potion. But now that shes a TOKEN slowly but surely her ORIGINAL body and health will come back to her, something that she is very happy about btw.
