Ian just wanted to leave the gang, which he did. But he also wanted to move away, which TJ didn’t want to see happen. So they’ve been in this dance where TJ does everything to keep him close and Ian stays when TJ steps away. He knows TJ needs him so he kind of likes it (as we can see in the flashback from chapter 15 when he smiles after TJ tells him he needs him), but at the same time he wants to be away from that environment.
In this chapter 45, Jo just realized that Ian is still in love with TJ but isn’t aware of it.
I think what the author is going for is that he does love TJ but Ian is so aware of how messy their relationship can be that he doesn’t think he does, which is why he referred to him as family. Because nothing else could explain the importance he has in his life. Basically the person who’s noticing that Ian still loves TJ here is Jo. However instead of being down about it, Jo is using the fact that TJ left Ian to tell Ian that he should respect TJ’s wishes and stay away.
Honestly i read this on and off and so now i’m just a bit confused and feel like I’m missing something. Was their history ever brought up properly? Does Ian not want to be with TJ because he doesn’t want the gang life anymore or does he think he’s bad for him? Or both ?
Sorry i’m dumb your insights would be appreciated
