Anyways, I'll come to that conclusion that he needs to save the fl but end up drowning and now he's in comma in his own world (let's assume fl is already safe) his soul somehow travel back time since he's in a comma. The reason why 28 years old Eisuke said they should break up (to their 18 years old selves) is probably because 28 yrs old Eisuke thought that he was hurting the fl with his childishness back then when him and fl are just 18, the only reason why present fl and 18 yrs old eisuke made up quickly is because of 28 years old eisuke.
