Kinda depends on if you watch the movies or the series. The series is kinda open ended and can be argued either way - Personally, I vote for the happy ending , especially considering that, I guess, the movie is more definite the he lives. I haven't seen the movie yet, so can't comment personally.

The whole anime is a fucking shipwreck, the ending it terrible, open ended or not, and you will be left in tears, crying out the frustrations of “hijikata why” and “i hate tuberculosis” and “what”, while you also simultaneously cry out your subconscious frustrations about how pathetic living is.
