well he did say he doesn't remember what he was like in real life, that can be either due to trauma of getting killed so many times or he really is just a character in game

With Doha, he's either the main character or related to the killer, and he also won autonomy.
There's no reality to come back to, they can only get away from repeating the same day over and over again.
And maybe Doha is responsible of bringing new victims to the relative, and kills any neighbour that appears during the blood-bath...
