It's possible to have penetrative sex without a lot of prep, I believe washing inside is more harmful than the act itself, but that really depends of each person and their own constitution, and like you said, how they are on the day. Sadly, the majority of straight men and tops irl also don't understand the most important thing about good sex is to focus on a lot foreplay, and that's way more about it than intercourse. Yah I m also reading yaoi/bl more than 9 years I m also straight myself so about finding my sexuality didn't come across my mind and i tend to enjoy story which is not straight, about those rape bl manhwa i assume even straight are same when it's written by men to rape women ( I'm talking about seinen plots) so both parties gives share amount of disgusting representation on each other that full fill in market to earn money, in between that reality overshadow the motive of an community that come between them is LGBTQ+ my male friend is gay he reads these stories he enjoy it but as fiction it's good people know difference between fiction and reality so they don't illusioned themselves thinking it's alright to happen to anyone in the first place , there are many people who read fucked up plot (I m also one of them) find peace in it but for some who sensitive to such things rather avoid it.

I also think that mangas are way ahead of manhwas but both are still mostly written for the fujoshis (straight female). I'm straight and don't really associate these works with IRL.
I also noticed that Korean manhwas are usually Cinderella stories so I appreciate it when the MCs are from the same socio-economic class which seems more common with Japanese mangas.

I think that depends on the person and their own biology. I've met people who did it almost every day and were completely fine. And I also had penetrative sex a few times without planning and encountered no accidents even though they didn't cleaned themselves beforehand.
