Even for humans, we have multiple stages of sex change, twice during conception (fertilization and then differentiation before embryo stage), then again during puberty where sex differences occur and are driven by hormones. This is how intersex people (people that are biologically neither male nor female) exist as they may hormonally, genetically, or physically be both sexes in various mixes; most are sterile but there are still those that can give birth even amongst intersex people.

As for trans, there is scientific evidence on gender dysphoria with brain scans even revealing that trans men and women have the brain for the opposite sex hence brain activity mismatching their biological body.
