I think it might have been rania's letters to him after the one she told him she was pregnant while he was away in battle. Since he believed she cheated on him he ignored her
That would be a parallel to what he's talking with the doctor, that if he doesn't protect her he can never make it up for it. He already feels as if he failed to protect her then and so cannot be excused
