Confused can someone clarify?

This confused me in the LN and the manga cause the way its depicted it seems like more than just physical abuse. It just seems so fucking weird like the way he abuses her, not to mention the fact that he's also in love with her. I feel like there are small things that made me question what's happening like the placement of her scars and how right in the beginning Ruby is sitting on his lap? Like juts the first chapter alone was ??? Moreover she seems deadset on keeping everything to herself which makes sense cause she was abused for so long and distrustful of anyone (rightfully so) but also she seems convinced that Izek won't love her or is scared that he won't if he finds out the truth. Which I feel wouldn't be so strong if it was just physical abuse. It's just so icky and for obvious reasons its not fully shown to us, idk I think if anything, it falls in a grey area.

When reading the novel, I always got the feeling that he was obsessed with her and had this no one hurt her unless I say or do so. Like, he would touch her shoulder that Iz touch as if he was covering (oveetaking) it with his touch and then asking if she slept with him. Enzo, was the normal one with a high stung personality, if I remember correctly hr didn't know what was going on.
