Jesse realizes nothing has been real. He's the one who has hemophilia. He wakes up in the “real” world where he is connected to a virtual reality machine named Moritat. Law starts to explain why he's done all this.

I'm still not sure who created Moritat.

I think the flashback conversation where Law tells Jesse he fiddled with their project was true. I think they were both working on a VR, but Law made it "Moritat" with the memory wipe and other features.
